+ Added \makeendnotes command to allow endnotes to be output
  in the case that alphabetical references (Author, Year) are 
  used.  In preprint mode, the endnotes are output in a separate
  section entitled "Notes".  In the galley mode, the endnotes
  are output in a separate section before the alphabetized list
  of references.  This makes the style file now fully compatible
  with the alphabetical style.  Note that the "endnotes" package
  must be employed for this to work properly.
+ Fixed bug in which the bibliography in galley mode was output 
  with font size \normalsize instead of \footnotesize.

+ Removed "Received:" from title page.
+ Removed "Short title:" from title page.
+ Added date to title page.
+ Added \ifoutputfig conditional.

KNOWN BUGS
-------------------------------------------------------------------
+ When using textual footnotes style, footnotes should be 
  intercalated and numbered consecutively in order of first 
  appearance.  This is contrary to REVTeX, which puts all the 
  footnotes at the end of all citations.  
+ In galley mode, the width of long affiliations isn't the same
  as the width of the abstract. 
+ The line spacing in the body of the text isn't quite double-spaced. 

Suggestions for fixes as are welcome.  Reports of bugs are also 
welcome (although more welcome if they come with fixes!).
